and operates housing communities in many areas of Maricopa County that are available
to qualified low-income residents.

Change
Form
This form is for Public Housing tenants to report changes to mailing adress, family
size, income and other information. Sign form and deliver to HAMC.
Change of Circumstance Form
This is a more formal, more detailed form
to report changes to tennant status, including names, marital status, family composition,
income and deductions. This must be signed and delivered to HAMC within 10 days.
This is important because it may impact your rent.
